To register at Yew Tree Medical Centre please check you are living within our catchment area, you can check this by clicking on the about us section.

Please note: You will need to bring you passport or another photographic ID and proof of address to register with the practice. For children under 16s we will need a copy of vaccination history.

Please complete the forms above and hand in to reception with your ID so we can take a copy for your medical records. It usually takes around 10 working days for us to process your registration. Once this has been done you will require a NHS new patient health check before you can make any GP appointments. You can arrange an NHS health check by booking via reception team. A New Patient Check is a check-up of your overall health. The answers and results can help your GP understand you better and address any current or future health issues. Once this has been done you are able to make any other appointments at the practice.
